Initial Comments:
Based on the findings of an off-site follow up survey conducted on February 27, 2023, Monarch Hospice had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare recertification survey completed November 17, 2022.
